WHOM 94.9 & Empire Beauty School's Contest for a full Esthetics Scholarship Entry Form

CONTEST INFORMATION:

1. Must be at least 18 years old to participate.
2. Must provide proof of a High School Diploma or GED Certificate prior to enrollment.
3. Must be a U.S. Citizen.
4. Full scholarship package must be used in one of the 5 class starts in 2008.
5. Full scholarship package includes: tuition, kit and books, lab supplies, and application fees with a value of $8750.00.
6. The offer has no cash value to the winner.
7. The contest is a random drawing of all eligible contestants. Drawing date is May 9, 2008.
8. Winner may use the full scholarship at either of the two Empire Beauty School's Educational Centers for Esthetics- Laconia or Portsmouth, N.H.
9. The scholarhip is not transferable to another person, and is valid only at the Portsmouth and Laconia campuses.
